How we care
Advanced primary care
Progressive Health brings more of the support needed for long-term health into one coordinated practice. Not a different kind of medicine. The same medicine, with more time and more people behind it.
Time
Standard primary-care visits are scheduled for 30 minutes. That is enough time to review your medications, talk about what is actually getting in the way, and leave with a plan you understand. Same-day sick visits are available for established patients, so an established patient with a new problem can usually be seen the same day.
A care team, not just a clinician
Your physician, nurse practitioner, or physician assistant leads your care. Around them are care navigators, certified health coaches, exercise professionals, registered dietitians, and the intensive cardiac rehabilitation team. Everyone works from the same record and the same plan.
Care navigators
Nurses and care staff who check in between visits, keep your information current, and coordinate next steps for patients enrolled in Care Management.
Health coaches
Certified coaches who help you turn a plan into habits and stay with you through setbacks.
Life-Fit assessment
A measured baseline of strength, endurance, and balance, with coaching toward practical goals.
Intensive cardiac rehabilitation
A Pritikin-model program in Wilmington for patients recovering from a cardiac event.
Follow-through
Most of what keeps people healthy happens between appointments. We use systems that help your team notice what may be due, keep your medication list accurate, and prepare for your visit before you arrive. Recommended screenings and follow-ups are less likely to be overlooked.
Technology helps your care team prepare, stay organized, and follow through. Your clinician reviews what applies to you and makes decisions with you. It never replaces clinical judgment.
Lifestyle medicine as part of the plan
Several of our clinicians are board-certified in lifestyle medicine. That means nutrition, movement, sleep, and stress are treated as clinical tools alongside medication, not as a pamphlet on the way out. When patients want to reduce medications, we build that plan together and measure whether it is working.
Progress, not perfection
We will not promise to reverse a disease or replace your medications. We will promise to know your goals, to notice when something changes, and to adjust with you.
