Advanced Primary Care · Wilmington & Newark
Primary care with more time, more support, and better follow-through.
Our clinicians schedule longer visits and work with care navigators, health coaches, and fitness professionals, supported by technology that helps your team stay current and notice what may need attention.
30-minute visits
Time to talk through what matters, not just the top complaint.
An assigned care navigator
For patients enrolled in Care Management, a named person who checks in between visits.
Nothing falls through the cracks
Systems that help your team notice screenings and follow-ups that may be due.
Deeper support when useful
Health coaches, Life-Fit fitness assessments, and intensive cardiac rehabilitation.
More support, not more pressure
We start with what matters to you, make a plan that fits real life, and help you adjust it over time. Progress may include medication, lifestyle changes, coaching, rehabilitation, or additional clinical care. Decisions are made with your care team.
- 1
Start with your goals
Tell us what you want your health to make possible.
- 2
Make a practical plan
Your clinician helps identify the medical care and everyday changes most likely to help.
- 3
Stay connected
Eligible patients get brief check-ins and care-management support between visits.
- 4
Adjust together
Your care team responds when something is not working instead of waiting for next year's visit.
Care Management
Support between visits
For eligible patients managing ongoing conditions, Care Management adds an assigned care navigator, brief check-ins, and remote monitoring when it helps. Your navigator keeps your plan current, spots problems early, and connects you with your clinician when needed.
Technology helps your care team prepare, stay organized, and follow through. Your clinician reviews what applies to you and makes decisions with you.
Is Care Management right for me?-
