For patients with ongoing conditions
Care Management
Ongoing support between visits from a named care navigator and your clinician. Not simply another appointment.
Ask whether Care Management is right for meWhat Care Management is
If you live with conditions like diabetes, high blood pressure, heart disease, COPD, or several at once, a lot can change in the months between visits. Care Management gives you a person whose job is to stay in touch, keep your plan current, and bring your clinician in when something needs attention.
Your assigned care navigator
A named member of our nursing and care team who knows your goals and your plan. They help maintain it, check on progress, coordinate referrals and follow-ups, and connect you with your clinician. You will know who they are and how to reach them.
How we stay current
- Brief check-ins by phone, portal, or the Progressive Health app covering medications, symptoms, appointments, goals, and barriers.
- Remote patient monitoring when it helps: a connected blood pressure cuff, scale, or glucose meter whose readings your team reviews.
- Medication reconciliation so every clinician you see works from an accurate list.
- A monthly review of your plan by your navigator, with your clinician looped in when something changes.
How we look ahead
Our systems help surface screenings, labs, and follow-ups that may be due, so recommended care is less likely to be overlooked. Your clinician decides what is appropriate for you.
Technology supports your care team's preparation and follow-through. It does not diagnose, prescribe, or replace a conversation with your clinician.
Deeper support when useful
Your navigator can connect you with health coaching, a Life-Fit fitness assessment, behavioral support through your care team, or intensive cardiac rehabilitation if you qualify.
Eligibility, consent, and costs
Who may qualify. Care Management is generally available to established Progressive Health patients with two or more ongoing conditions expected to last a year or more. Medicare and many other plans cover it. Your care team will confirm eligibility with you.
It is optional. Enrollment requires your consent, and you can stop at any time.
Cost sharing may apply. Medicare and some plans treat Care Management and remote monitoring as covered services with a monthly copay or coinsurance, depending on your coverage. We will explain what to expect before you enroll. See insurance and costs.
Ask whether Care Management is right for you
Current patients: send a message through the patient portal or ask at your next visit. Not yet a patient: become one first, and your clinician will discuss it with you.
Please do not send medical details by regular email. The portal is the secure channel.
