We accept Medicare and all major local insurance plans. Here is what to expect before you enroll.
Plans we accept
We participate with Medicare, Medicare Advantage plans, and the major commercial insurers serving Delaware. Plan networks change, so please call your office with your plan name and member ID before your first visit and we will confirm. Wilmington: 302-543-5454. Newark: 302-731-0900.
What may involve cost sharing

| Annual wellness visit | Covered without cost sharing by Medicare and most plans when scheduled as a preventive visit. Problems addressed at the same visit may be billed separately. |
| Care Management | Covered by Medicare and many plans as a monthly service. Medicare applies coinsurance unless you have supplemental coverage. We explain this before you consent. |
| Remote patient monitoring | Covered by Medicare and many plans; coinsurance may apply. |
| Intensive cardiac rehabilitation | Covered by Medicare for qualifying diagnoses. Commercial coverage varies. We verify benefits before you start. |
| Health coaching and Life-Fit | Coverage varies. Some coaching is included in Care Management or ICR; standalone sessions may be self-pay. Ask before scheduling. |
| Telehealth | Generally covered like an office visit. |
Questions before you enroll
Call your office and ask for billing. For intensive cardiac rehab, call the program directly at 302-309-1719. We would rather answer a cost question before your visit than surprise you after it.
