Portal or app?

The patient portal (Elation Passport) is for appointments, records, messages, and refills. Every patient has access. The Progressive Health app is for patients enrolled in Care Management: guided forms, brief check-ins, and care-plan information. It is powered by Truer Health and supports your care team; it does not replace them.

Urgent concerns and emergencies

Call 911 for chest pain, trouble breathing, signs of stroke, severe bleeding, or any emergency. Do not use the portal, email, or a form.

Mental health crisis: call or text 988 any time.

After hours: call your office number and follow the prompts to reach the on-call clinician for urgent issues that cannot wait until morning.

Where not to send medical information: regular email (including care@ and fit@ addresses) and social media are not secure and are not monitored for clinical messages. Use the portal.
